In Attendance (via phone):
* Randy Gollub
* Carolyn Zyloney
* Sarah McMordie
* Bill Hanlon
* Gordon Williams
1. Report from the CTSA Working Group Bi-Monthly Call (October 20th, 2008)
* Dan Sullivan led the t-con. Ron Kikinis, Bill Hanlon, and Randy Gollub were present and represented the Harvard CTSC.

*Bill Hanlon from the TIMC has been invited to the November 7th National CTSA Image Informatics meeting in Chicagoland and will represent the Harvard CTSC at this meeting.

* The goal of the meeting is to work out the details of the grant proposal to support medical image informatics.
** Bill Hanlon is charged with sharing the details of i2b2 and XNAT to see how it will fit into the national CTSA informatics plans.

* One topic of discussion at the CTSA t-con was the idea of setting up standardized measurements and standardizing the cost of image acquisition and analysis. This has spurred interest in how the TIMC does business.
** Currently the TIMC (Tumor Imaging Metrics Core) offers its data analysis services to trials within the DF/HCC.
** The November 7th meeting in Chicago is one potential opportunity for Bill to share information about the TIMC with other CTSCs.
** Before we proceed with sharing the strategies employed by the TIMC with the world, the TIMC PIs will decide what information they wish to make publicly available.

* Bill Hanlon has been invited to present information about the TIMC at the RSNA meeting.

* IRAT, the Imaging Response Assessment Team, has the goal of providing funding to encourage clinical collaboration between imaging scientists and oncologic investigators at Cancer Centers to identify new oncologic imaging research opportunities in clinical trials that warrant multi-center clinical investigations and integrate imaging data as potential biomarkers or candidate surrogate markers in clinical therapeutic trials. IRAT is entering its third year of funding, but is currently looking for other sources of funding to support its work.

* Few other CTSCs have as well establish and broadly knowledgeable senior leadership as the Harvard CTSC. This puts the Harvard CTSC in a leadership position.
2. Communication with Imaging Community
* The Imaging Consortium has obtained the slides from the CTSC retreat.
* The Imaging Consortium will work together at the next meeting (October 28th at Harvard Medical School Countway Library) to prepare additional slides about the Imaging Consortium and its consultation service.
* The draft of the presentation will be sent to Consortium members via email prior to the October 28th meeting.
* Consortium members will use this PowerPoint to present the CTSC Imaging Consultation Services at the Radiology Division Heads Meetings within each institution.
3. Imaging Navigator Job
* Randy has begun to receive resumes from prospective Imaging Navigators
* Carolyn has contacted the HR departments at Harvard and MGH to post the Imaging Navigator job.
* Consortium members and personnel within the Radiology Departments at each institution may be able to suggest suitable job candidates.
* Carolyn has also submitted the job description to RSNA. It will be posted on the RSNA website and featured in the December issue of Radiology.
* Randy requests the assistance of consortium members at other institutions to help get the job description posted there through each institutions respective HR department.
4. 2009 meeting schedule
* Randy proposed that the Imaging Consortium continue with the current meeting plan into 2009. This includes continuing our 8:30 - 9:30 am weekly teleconferences and our monthly site visits at Harvard CTSC Central or one of the participating institutions.
5. Consultation Web-page Update
* The Imaging Consultation Service page is due to go live on the Catalyst website on November 18th.
* We are waiting for CTSC Central to approve rotating through several images at the top of the web-page. If it is approved, consortium members will be asked to suggest several additional pictures to display on the website.
Harvard CTSC Medical Imaging Consultation Program
Welcome to the Consultation Unit of the Harvard Catalyst Translational Imaging Service. The Consultation Service is designed to assist clinical and translational investigators in the planning and design of medical imaging research. The Consultation Unit includes faculty from across the Harvard affiliated medical centers with expertise in the acquisition, analysis and interpretation of medical imaging data.

For more information, or to request a consultation, please contact:
1. What kind of consultation is available?
We will work with you to address your individual needs related to the design and planning of medical imaging research. Common requests for consultation include:
* selection of appropriate imaging modality to assess clinical or treatment outcome
* choice of appropriate image acquisition parameters for greatest sensitivity and specificity of outcome measure
* identification of most appropriate medical image analysis methods (including medical imaging statistics)
* development of experimental design for clinical translational studies
* identifying appropriate imaging acquisition and analysis facilities for clinical trials (e.g. DFCI Tumor Metric Core)
* identifying appropriate collaborators for imaging and image analysis
* guidance for implementation of multi-site studies with medical image acquisition
* training in the use of specific medical image analysis software tools (e.g. Slicer 3, Freesurfer)
* guidance on IRB and human subjects issues specific to medical imaging studies including template language
* career development guidance for investigators in the areas of biomedical imaging
2. Who is eligible to receive consultation services?
Consultation is available for clinical and translational investigators in the Harvard University community (postdoctoral fellows and faculty members).

3. What kind of consultation is not available?
Our consultants serve the role of advisor and facilitator; they are not key personnel on projects. Specifically,
* We can provide general guidance for projects that involve statistical analysis of medical imaging data but consultation regarding substantial statistical services (power calculations, data analysis, etc) can be obtained through the CTSC Biostatistics consultation service.
* We are focused on assisting with the planning and design of research proposals and/or methods for image analysis and do not provide ongoing support for the implementation of new studies or the analysis of data from existing studies. Such “post-funding” research expertise will normally be covered by key personnel on the specific project funded by the investigator. We can however assist in the identification of appropriately trained individuals who are available for collaboration or service work (networking). Thus, for example, consultation is available for the preparation of a grant or IRB submission, but is not designed to support the implementation or analysis of a funded project. We can also assist in the preparation of Catalyst Pilot grants that request funds for the acquisition and/or analysis of medical imaging data.
4. What costs are associated with consultation?
*Consultation is provided free of charge to Harvard investigators.
5. How do I request a consultation?
* To initiate a consultation, download and complete the Request for Imaging Consultation Form. Email the form to the triage individual named above. After your request is submitted, it will be reviewed by the consultation service and you will be contacted to arrange the details of the consultation. In some cases, we may contact you to clarify the nature of your request.

Consultation is available for clinical and translational investigators in the Harvard University community (postdoctoral fellows and faculty members).
* We can provide general guidance for projects that involve statistical analysis of medical imaging data but consultation regarding substantial statistical services (power calculations, data analysis, etc) can be obtained through the CTSC Biostatistics consultation service.
* The Consultation Service is focused on assisting with the planning and design of research proposals and does not provide ongoing support for the implementation and analysis of existing studies. Such “post-funding” research expertise will normally be covered by key personnel on the specific project. Thus, for example, consultation is available for the preparation of a grant or IRB submission, but is not designed to support the implementation or analysis of a funded project.
4. What costs are associated with consultation?
*Consultation is provided free of charge to Harvard investigators.
5. How do I request a consultation?
* To initiate a consultation, download and complete the request for consultation form. Email the form to the triage individual named above. After your request is submitted, it will be reviewed by the consultation service and you will be contacted to arrange the details of the consultation. In some cases, we may contact you to clarify the nature of your request.
1. Consultation Service
The draft of the Imaging Consultation web-page has been posted on this wiki. The Imaging Consortium unanimously approved of the content of this draft but with the following improvements suggested.
* Zeke and colleagues are to draft the top-level text of the Imaging consultation web-page so that it is in harmony with the other consultation web-pages on the Harvard Catalyst website.
* Zeke has created a draft of the . This document is similar to the consultation request forms already posted on the Biostatistics and Genetics consultation web-pages on the Harvard Catalyst website, but includes a list of consultation services that are specific to the Imaging group. Investigators will be able to download and submit the form. This request form will be discussed at the next imaging teleconference. The proposed web-based consultation request system with RSS feed will not be available for the first release of the consultation web-site but the Imaging Consortium still plans to develop this submission system.
* All Imaging Consortium sites are requested to refine the list of expertise at their site to include all available imaging modalities that the consortium members possess expertise in as well as any expertise in the areas of image acquisition, analysis, and experimental design.
* The Imaging Information and Analysis Liaison/Imaging Navigator/Triage Consultant job description will be posted by the next teleconference for discussion.
The Imaging Consortium plans on including this consultation service web-page with the next Harvard Catalyst web-site release on October 15th.

2. Image Analysis
* Annick commented and the group agreed that image acquisition is related to its analysis and both aspects of bioimaging research are equally important.
*The wiki page should be updated to include each site's analysis expertise.
**Randy Gollub will talk with Bruce Fishel who is an analysis expert at MGH.
**Ron Kikinis and Clare Tempany represent the analysis expertise at BWH.
**Simon Warfield represents the analysis expertise at Children's Hospital.
**Gordon Harris is an analysis expert at DF/HCC (MGH)
**The Consortium requests an update on the analysis expertise at BIDMC
* Consortium sites have strong expertise in medical image software tools, many of which were internally developed and are internationally used. Analysis tools expertise at the consortium sites include:
** Freesurfer - MGH
** Slicer - BWH
In addition to these tools, the consortium possesses expertise in other externally developed analysis tools.
* The consortium possesses expertise in multi-site imaging studies and imaging projects that include image acquisition at one site and analysis at another site.
*It was proposed that the web-site can include a web based repository of suggestions based on clincial scenarios ("case studies"). Further discussion of this suggestion is on the agenda for a future teleconference.
* There is the possibility that, through collaboration and working with the existing imaging analysis services at our sites, the Imaging Consortium could potentially provide image analysis services to investigators. CTSC funds will not cover this service. Further discussion is needed to outline the means of supporting the costs of this service.

3. Update on Pediatric Neuroimaging
* Stephan Voss indicated that there is a great demand for more pediatric clinical imaging research but the resources at Children's Hospital are limited. The group supports pediatric imaging acquisition at sites that have the existing infrastructure to support it.
* Dr. Ellen Grant, who has been working with pediatric neuroimaging at sites within the consortium has agreed to provide consult services at the CTSC. Randy Gollub will meet with her and Rudolph Pienaar, the medical imaging and informatics expert, this week to learn more about the current status of Dr. Grant's support and how we can be most helpful going forward.
* Dr. Grant and Rudolph Pienaar have already begun to use XNAT as an imaging research data repository for multi-site projects and have image analysis workflows in place that would provide templates for other users. They can collaborate with the Imaging Consortium to share these resources.

4. Informatics Update
The proposal is currently under consideration at CTSC central. Bill Hanlon agreed to prepare additional text to outline specific details of transferring image data from each hospital's PACS system to an image repository, including the current status and problems and suggested solutions. Randy Gollub is working to collect driving biological projects and to identify and write up a list of initial clinical translational investigators to work with the Imaging Consortium to pilot the first use of these resources.
* Note: Jeff Yap circulated a (National Cancer Institute) that includes information on translational medical imaging. There was no time to discuss the article during this teleconference. The points from this manuscript pertinent to the Imaging Consortium can be raised at next week's teleconference.
* identifying appropriate imaging acquisition and analysis facilities
* identifying appropriate collaborators for imaging and image analysis
* guidance for implementation of multi-site studies with medical image acquisition
* training in the use of specific medical image analysis software tools (Slicer 3, Freesurfer, ?)

* Zeke Benstein-Hanley
*Stephan Voss
**Topics
Gordon Williams outlined 4 milestones for 9/1/08
*1. Resources: must be populated by each site in this wiki.
*2. Consultation: How do investigators access the program-the service plan must be available electronically in CONNECT
*3. Services: Determination of the servcices to be provided by the imaging core
*4. Education program outline and description with dates of 3-4 events per year again posted on CONNECT.
Budget discussion
This was discussed by all after an outlined -below- provided by Gordon W.
3 aspects to budget as it now stands- $1.2M
* Consultation service- expected to be provided through the funds already allocated in the grant for the associated directors- all of us- at each site to provide this service
* Need to determine how much help we can provide after initial consultation- with no $$ for scan time- before investigators move to apply for CTSC pilot funds

** Pilot funds- how best to optimize this opportunity- the "GW" bias-pilots with collaborative teams of a junior imaging investigator paired with CTSC investigator -cross disiplinary projects will enhance chances of success. There will be a CTSC study section- not yet in place- they will review all pilot applications.

**Bruce Rosen proposed that we think through how best to use the funds in one of 2 possible ways
* expand the pool of funded individuals at each site
* considered hiring cross insitutional individuals who will provide a unique resource -not already available for the core and all of the CTSC investigators, examples included- standarization of imaging protocols- such as anatomical sequences, database establishment and management.

*Internal timetable to meet goals - proposed by the group that in next 2 weeks we try to achieve the resources, education and consultation goals
* considered hiring cross insitutional individuals who will provide a unique resource -not alread available for the core and all of the CTSC investigators, examples included- standarization of imaging protocols- such as anatomical sequences, data base establishment and management.

*Internal timetable to meet goals- proposed by the group that in next 2 weeks we try to achieve the resources, education and consultation goals
The Dana-Farber / Harvard Cancer Center (DF/HCC) Tumor Imaging Metrics Core (TIMC) is a multi-institutional, multi-disciplinary imaging core laboratory co-directed by Gordon J. Harris, Ph.D. (MGH), Annick D. Van den Abbeele M.D. (DFCI), and Matthew A. Barish M.D. (BWH). The TIMC provides standardized tumor measurements of radiology scans for DF/HCC clinical trials. The TIMC maintains laboratory space at several locations in the Boston area to provide the clinical image analysis and scan review services.

At Massachusetts General Hospital: 320 sq. ft. including one office and four cubicles in a central work area at 25 New Chardon Street, and 64 sq ft in the radiology reading room (Dodd Room).

At Brigham and Women’s Hospital: 64 sq. ft. work area within the radiology 3D Image Processing Center, and 1,200 sq. ft. of shared office and cubicle space off the main campus in the Crosstown satellite facility at 801 Mass Ave.

At Dana-Farber Cancer Institute: 64 sq. ft. work area in the Radiology department reading room for clinical scan review.

The TIMC operates both Unix and PC-Windows XP-based software applications for the analysis of CT, MR and PET images. The TIMC lab computers include three Hermes image processing workstations, two Hermes image archives 1.2TB each (mirrored), two web servers, two shared image archives, 600MB each (mirrored), four Voxar 3D image processing workstations, five general-purpose PCs and an additional 2TB of image storage. All computers are connected with the hospital network at 1Gb speed with DICOM and LAN-to-LAN connections to the PACS at MGH, BWH, DFCI and BIDMC.

TIMC services are available to all DF/HCC and non-DF/HCC oncology PIs on a fee-for-service basis. Complete information on the DF/HCC Tumor Imaging Metrics Core, including fee structure, can be found on the TIMC website www.tumormetrics.org.
